Family Inquiry Information
The family inquiry form may request contact, demographic, insurance, and limited clinical intake information so HIPD can review service needs and coordinate next steps. Please provide only the information requested by the form. Do not upload or transmit medical records, diagnostic reports, or other documents through free-text fields unless HIPD provides an approved method for doing so.
